GABA — your nervous system’s brake

People who regularly drink alcohol have been shown to have lower GABA in key brain regions. GABA is the brain’s natural brake — it helps quiet racing thoughts, reduce anxious arousal, control impulsiveness, and keep the nervous system from running too hard. Alcohol temporarily boosts that braking system. That is one reason a drink can feel like it takes the edge off.

Baclofen is a GABA-B agonist. It activates GABA-B receptors and effectively gives that braking system more help — so you no longer need alcohol to do the same job. Baclofen vs naltrexone

Individually titrated baclofen

All were treatment-resistant heavy drinkers and high-risk at baseline. None was in remission.

Baclofen stabilizes GABA pathways, which acts as a natural brake system to control feelings of anxiety, fear, stress, and nervous system overdrive. It also helps the brain relax before sleep and regulates deep sleep alongside circadian rhythms. For people whose drinking is driven by anxiety, racing thoughts, or difficulty switching off, it addresses that driver BEFORE the craving-and-reward cycle takes over. It treats the CAUSE of drinking.

Naltrexone, on the other hand, kicks in AFTER the craving, willpower struggle, anxiety, and drinking have started by treating the EFFECT of drinking. The brain registers the alcohol, but not the chemical euphoria. It takes away the “buzz”, enjoyment, and reward of drinking alcohol. Over time, with a strict regimen taken before drinking EVERY time, this may induce pharmacological extinction.
